Cracked/Broken Exhaust Risers

During annual inspection, found muffler end where riser attaches to be broken/cracked. Further examination found riser cracked (very tight) at weld joint.

Hole In Tailpipe

During morning servicing of aircraft before being removed from the hangar, the technician noticed discoloration on tail pipe. Soot was cleaned off to reveal a small hole had burned through tail pipe. Exhaust was replaced with new unit.

Failed Spotwelds

During routine inspection, technician noted two spotwelds on the shroud support flange had failed, resulting in open holes in the muffler can. Replaced muffler with new unit.

Carbon Monoxide Warning

Crew received CO warning in flight and made a precautionary landing. Upon visual inspection of exhaust system, a crack was found in the aft muffler where it transitions from the forward muffler. The muffler was replaced; no CO warning was noted on run up.

Blocked Tailpipe

The crew reported the right engine was running rough in cruise flight. Maintenance found that the right engine forward muffler tailpipe was almost completely blocked with the internal pipe jammed into it. A new muffler was installed.

Deteriorated Ball Joints

While complying with AD 2015-02-09, found exhaust pipes leaking at the ball joints where they connect to the muffler. One side of a ball joint lip on the muffler had deteriorated substantially, from about 0.065 in to 0.023. All ball joints showed signs of leaking and heat corrosion.

Failed Spotwelds

Owner reported high CO values on cockpit detector. Removed muffler shroud and found the six tack welds attaching the muffler shroud support ring had failed/broken. This weld failure created a hole in the muffler.
